Referring physicians are considered an integral part of the Texas Transplant Institute’s medical team and will be involved in all phases of the patient’s plan of care.

Physicians who have a patient that is not responding to medical management of end-stage organ failure, or is a potential candidate for a solid organ or stem cell transplant can initiate the referral process by contacting us at numbers listed below:

After the referral information has been received by our transplant clinic, financial clearance will be verified by a financial coordinator and an appointment will be set by the clinic secretary. The patient and doctor appointment notification letter will then be sent.

After the initial referral call, the following patient information will need to be faxed or mailed to our clinic:

  • patient name, DOB, SSN, address, phone numbers, and insurance information
  • diagnosis
  • a complete physical and disease history to include surgeries and/or procedures
  • current list of medications to include types, dosages, and any allergies
  • all pathology reports, slides, lab reports
  • therapy summary/flow sheets
  • CT and x-ray reports
  • any additional information available

And the following process begins:

    • Our financial coordinator verifies insurance benefits and creates a financial plan regarding services.
    • A specialist contacts the patient and referring physician’s office to discuss expectations and to schedule an appointment for a medical evaluation with the multi-disciplinary team of experts.
    • At the initial visit, the patient meets with the medical team including the physician, nurse, dietitian, and social worker.
    • Following the initial visit, a report is mailed providing our team’s recommendation whether to proceed with the transplant process, or end-stage organ failure medical management.
    • Following each patient clinic visit, dictated reports of all test results and physician recommendations are mailed.
    • During medical management or after transplant, our physicians work closely with referring physicians to provide the best method to monitor the patient’s progress. The primary care or referring physician remains responsible for the patient’s general health.
